Consumo de amitriptilina en pacientes atendidos en el Consultorio # 18 de la localidad de San Juan de Amaro

Resumen
La depresión es un trastorno del estado de ánimo caracterizado por sentimientos de tristeza, inutilidad, culpa y desesperanza. En Cuba, el 5.5 % de la población padece de esta enfermedad. Algunos estudios sobre la utilización de antidepresivos (ADs) demostraron que en la última década del siglo XX hubo un incremento progresivo en el consumo de estos fármacos; situación similar a la observada entre julio de 2020 y junio de 2021 en el Consultorio #18 de la localidad San Juan de Amaro, específicamente de la amitriptilina. Para conocer las causas de dicho aumento se diseñó un estudio descriptivo, observacional y retrospectivo incluyendo 43 pacientes mayores de 34 años que presentaron padecimientos relacionados con la prescripción del fármaco. Se analizó el consumo y las indicaciones de amitriptilina en el periodo, se evaluó el nivel de conocimiento de los pacientes respecto al fármaco y se desarrolló una intervención educativa para mejorar el conocimiento acerca del mismo y contribuir así a su uso racional. En el período evaluado se consumieron 28.99 DHD de amitriptilina por cada 1000 habitantes, siendo indicado principalmente para el síndrome ansioso depresivo, los dolores musculares y el insomnio. El incremento en el consumo estuvo marcado por la falta de disponibilidad de otros medicamentos relacionados con la indicación del fármaco, la prescripción inadecuada del mismo, la automedicación y el aumento de los padecimientos ansiosos producto del confinamiento social a causa de la COVID-19. Se comprobó un relativo desconocimiento por parte de los pacientes respecto al medicamento que mejoró tras la intervención educativa realizada.
Depression is a mood disorder characterized by feelings of sadness, worthlessness, guilt, and hopelessness. In Cuba, 5.5% of the population suffers from this disease. Some studies on the use of antidepressants (ADs) showed that in the last decade of the 20th century there was a progressive increase in the use of these drugs; similar situation was observed between July 2020 and June 2021 in the Clinic # 18 in the town of San Juan de Amaro, specifically amitriptyline. To find out the causes of this increase, a descriptive, observational and retrospective study was designed including 43 patients over 34 years of age who presented conditions related to the prescription of the drug. The consumption and indications of amitriptyline were analyzed in the period, the level of knowledge of the patients regarding the drug was evaluated and an educational intervention was developed to improve knowledge about it and thus contribute to its rational use. In the evaluated period, 28.99 DHD of amitriptyline were consumed per 1000 inhabitants, being indicated mainly for anxiety-depressive syndrome, muscle pain and insomnia. The increase in consumption was marked by the lack of availability of other medications related to the indication of the drug, its inappropriate prescription, self-medication and the increase in anxiety disorders as a result of social confinement due to COVID-19.
