Seguridad y efectividad de la vacuna CIMAvax-EGF en el tratamiento de pacientes con cáncer de pulmón de células no pequeñas en estadios avanzados

Resumen

Introducción: Ensayos clínicos previos han demostrado la eficacia de la vacuna CIMAvax-EGF® en el tratamiento de pacientes con cáncer de pulmón avanzado y su relación con las concentraciones basales de EGF. Este trabajo tiene como Objetivo: Evaluar la seguridad y efectividad de CIMAvax-EGF® para el tratamiento de pacientes con cáncer de pulmón de células no pequeñas en estadios avanzados en las condiciones de la práctica médica habitual y su relación con las concentraciones basales de EGF. Método: se realizó un ensayo clínico abierto, no comparado, multicéntrico, fase IV, entre enero de 2016 y agosto de 2021 en Villa Clara. Se incluyeron 127 pacientes no elegibles para recibir quimioterapia o radioterapia, o aquellos que la recibieron y no tenían otra opción terapéutica. Se evaluó la seguridad y la supervivencia global. Resultados: Los eventos adversos más frecuentes fueron grado 1 y 2. La mediana del tiempo de supervivencia global (mSG) para todos los pacientes vacunados fue de 6.30 meses y de 7.75 meses para aquellos que recibieron 4 dosis de la vacuna. Para los pacientes con concentraciones de EGF inferiores a 870 pg / ml la mSG fue de 6,3 meses y en superiores, de 3.94 meses, sin diferencias significativas (p=0.818). En el subgrupo de pacientes que recibieron al menos 4 dosis de CIMAvaxEG, la mSG para pacientes con EGF bajo fue de 7.58 meses y para EGF alto fue de 9.65 meses, sin diferencias significativas (p=0.976). Conclusiones: la administración de CIMAvax-EGF es segura al ser administrada en las condiciones de la práctica médica habitual. Los pacientes con EGF alto y mal pronóstico logran supervivencias comparables a aquellos con EGF bajo y un mejor pronóstico de la enfermedad en términos de supervivencia. Los pacientes que recibieron al menos 4 dosis del tratamiento reciben el mayor beneficio
Introduction: Previous clinical trials have demonstrated the efficacy of the CIMAvax-EGF® vaccine in the treatment of patients with advanced lung cancer and its relationship with baseline EGF concentrations. This work aims: to evaluate the safety and effectiveness of CIMAvax-EGF® for the treatment of advanced non-small cell lung cancer patients under the conditions of normal medical practice and its relationship with basal EGF concentrations. Method: an open label, non-compared, multicenter, phase IV, clinical trial was conducted between January 2016 and August 2021 in Villa Clara. 127 patients, ineligible for chemotherapy or radiotherapy, or those who received it and had no other therapeutic option, were included. Safety and overall survival were evaluated. Results: The most frequent adverse events were grade 1 and 2. The median overall survival time (mSG) for all vaccinated patients was 6.30 months and 7.75 months for those who received at least 4 doses of the vaccine. For patients with EGF concentrations lower than 870 pg / ml, the mSG was 6.3 months and in higher ones, 3.94 months, without significant differences (p = 0.818). In the subgroup of patients who received at least 4 doses of CIMAvaxEG, the mSG for patients with low EGF was 7.58 months and for high EGF it was 9.65 months, without significant differences (p = 0.976).
